Portal Privacy Notice

This Portal Privacy Notice (“Privacy Notice”) applies to GHI’s collection, use, storage and disclosure of personal information through our Portal, and your choices and obligations with regard to that information. This Privacy Notice is incorporated into our Portal Terms of Use, as well as our GHI Privacy Policy. This Privacy Notice uses terms defined in those agreements. If this Privacy Notice conflicts with our GHI Privacy Policy, this Privacy Notice will control.

By accessing our Portal and using the Services, you agree to the terms of this Privacy Notice.

How we collect and use information

WHEN YOU REGISTER

We may collect information from users of the Portal, including Physicians and Delegates. We collect information including your name, email address, password, security question responses, and other registration information. We use this information to create user accounts, contact you about issues with your account, and provide other information we believe is important to you.

AS PART OF A DIAGNOSTIC TEST OR OTHER PATIENT SERVICE YOU REQUEST

The Portal is used for the storage and transmission of Protected Health Information (also referred to as “PHI” and as defined under HIPAA) between GHI, Physicians and their Delegates. We will process PHI when you request Services through the Portal. We use PHI only for the purposes you authorize, and otherwise in accordance with applicable law. Please see our Notice of Privacy Practices for more information about our handling of PHI.

WHEN YOU SEND US AN EMAIL

If you contact us about the Portal or Services, we may retain the content of the email, your email address, and our response in order to process your request. Please note certain Services may require a valid email address in order to register and/or continue use of the Service. You will receive email alerts or notification messages from these programs unless you unsubscribe from these Services. WHEN YOU USE CERTAIN FEATURES OF THE PORTAL

If you use certain features of the Portal, such as the Address Book or the Calculation Tools, you will enter additional information about yourself, third-party contacts, and PHI (in the case of the Calculation Tools). We process such information only to provide you with access to the feature you have requested.

How we share information

We do not sell your personal information to any third parties, and share it with our third party service providers only to provide you with the Services. We only share PHI in accordance with our Notice of Privacy Practices. We may disclose information about you if we are required to do so by applicable law or legal process (including to law enforcement authorities if required), or when disclosure is necessary to prevent physical harm. We reserve the right to transfer any information we have about you in the event we merge with another organization, or sell or transfer all or a portion of our business or assets (including through bankruptcy).

How we secure information

Our goal is to protect information you submit to us through the Portal. We secure information contained in the Portal, including PHI, using administrative, physical and technical safeguards. For example, we encrypt information in transfer using Secure Sockets Layer (SSL) technology. We store information on controlled servers with restricted access. All access is password protected and each individual user has his/her own user name and password. GHI tracks and audits access to sensitive information in accordance with its security program.

How you may access information

We limit access to a patient's results to the patient, the patient's Physician, or a designated Delegate of such Physician. GHI will permit access to a patient's specific data by a non-designated, participating physician through the Portal only if the data is de-identified consistent with HIPAA and anonymized as required by applicable foreign data privacy laws. Our Notice of Privacy Practices provides more information about our use and disclosure of confidential information and PHI.

Updates

We may update this Privacy Notice from time to time to reflect changes and new features and practices. We will update the “Last Updated” date, above, accordingly. We also encourage you to review this Privacy Notice periodically to understand how GHI protects and uses your information.
