1. Contact Genomic Health's® Customer Service at
customerservice@genomichealth.com or by calling 001-650-556-9300,
option 1, to receive specimen transportation boxes, requisition forms and
pathology guidelines for preparation of samples.

2. When you are ready to submit your patient's specimen, complete the Oncotype
DX requisition form and send it along with the specimen. Pre-payment is
required prior to the release of results. Acceptable forms of payment include
credit card, international money order in U.S. dollars, or bank wire transfer.
Please contact Customer Service for instructions on wire transfers.

3. Pathologist retrieves and prepares specimen for submission. Detailed
instructions are provided in the specimen transportation box. Genomic Health
prefers to receive prepared unstained slides from the tumor block or the tumor
block itself embedded in paraffin. If submitting the tumor block, please be
advised that Genomic Health will not be responsible for any specimens retained
or fees imposed by customs in your country during specimen return.

4. Send the specimen in the Oncotype DX specimen transportation box to Genomic
Health via our preferred courier service, Federal Express, using the
pre-printed shipping document. A Customs Declaration and commercial invoice are
required for the specimen to be accepted into the United States. Please include
these documents along with the Federal Express Airway bill. Genomic Health
covers the shipping charges to send the specimen from your country to the
United States.

5. Results are returned via online secure access, fax and priority mail, within
10 to 14 calendar days of specimen receipt. |
