Pathology and Laboratory Professionals: Submit a Sample

The Oncotype DX® sample requires either:

Two microcentrifuge tubes, each containing three 10-micron sections of fixed paraffin-embedded breast tumor tissue, and one H&E slide from the same block. Note: Reimbursement is available from Genomic Health® for preparation of this type of specimen. Please call Customer Service at (866) ONCOTYPE (866-662-6897) for details.
OR
One fixed paraffin-embedded breast tumor block


All specimens must be labeled with barcode labels from the Oncotype DX Requisition Form for that patient.

Additional information for the preparation of tumor specimens for microcentrifuge tubes:

  1. Gently grasp the first section with the appropriate implements.
  2. Gently place each rolled-up section of tumor tissue into the microcentrifuge tube.
  3. Gently push each section into the microcentrifuge tube. Tighten the cap on the tube.

Oncotype DX Requisition Form Instructions

When submitting an Oncotype DX sample, please fill out each section of the Requisition Form. Attached is an example of this form, as well as instructions for completing the form.
