1. Authorized healthcare provider completes a Genomic Access Program (GAP) Benefits Investigation / Statement of Medical Necessity Form, if desired |

2. Authorized healthcare provider completes a Pathology Pre-Order Form OR fills out an Oncotype DX® Requisition Form |

3. Pathologist retrieves and prepares specimen for submission (Click here for information on specimen submission) |

4. Pathologist completes the Oncotype DX Requisition Form and sends the Specimen Transportation and Collection Kit to Genomic Health® |

5. Results are returned via fax, overnight mail, or secure online access, within 10 to 14 days of specimen receipt |

6. Genomic Health submits Insurance Claim (Click here for information on Health Insurance Coverage) |

Oncotype DX Report – Sample
An Oncotype DX Report is generated for each patient, with assay results provided as a Recurrence Score® (0-100). The Recurrence Score (RS) correlates with a specific likelihood of distant recurrence at 10 years, presented as the average 10-year distant recurrence rate for that RS in the clinical trial population. There are now two versions of the Oncotype DX report: a node negative and a node positive report.
